Building Digital Legacies: What Ancient Engineering Taught Me About #AI

Most healthcare AI systems won’t last 10 years.

Standing in front of #Petra, that realization hit me hard.

This structure was carved directly into stone more than 2,000 years ago.
No software updates.
No quarterly roadmaps.
No rush to deploy.

It wasn’t built for speed.
It was built to endure.

In healthcare AI, we’re in our own carving phase right now.

We’re shaping algorithms, data pipelines, and digital infrastructure that will define how medicine is practiced long after we’re gone.

But here’s the uncomfortable question:

Are we building for longevity or just for launch?

• Are our data foundations solid enough to evolve?
• Are ethics designed in, or patched on later?
• Will these systems still deserve trust when incentives change?

The Nabataeans understood something we rarely talk about in AI:

Alignment between builders, purpose, and consequences.

Everyone keeps saying:

“Don’t worry, there’s a human in the loop.”

That phrase is becoming dangerously meaningless.

In theory, a human in the loop means:
A person understands what the AI is doing
Can intervene in time
And has real authority to stop it

In practice, here’s what it often means:
• The AI moves faster than human review
• The human rubber-stamps outputs they didn’t generate
• Oversight exists on paper, not in reality

Medicine has seen this before.

When alerts fire constantly, clinicians stop *seeing* them.
When systems feel reliable, supervision quietly erodes.
Not because people are careless because cognition is finite.

AI didn’t remove humans from the loop.
It **thinned the loop** until it became symbolic.

The uncomfortable question leaders need to ask:
Is your human actually supervising  or just absorbing liability?

Real oversight requires:
Time to understand decisions
Interfaces designed for comprehension, not speed
Clear moments where AI must pause and wait
And explicit ownership when things go wrong

If your AI can act faster than a human can meaningfully intervene,
you don’t have a human in the loop.

You have a **human on the receipt**.

The next phase of AI maturity won’t be about smarter models.
It will be about **honest governance**.

This isn’t a chatbot update.
It’s the beginning of the patient digital twin.

But that framing misses what’s actually happening.
For the first time, a mainstream AI is being positioned as a persistent health intelligence layer.

In medicine, we know the difference immediately.
Tools answer questions. Systems carry memory.
And memory is where healthcare changes.

When an AI can:
• Understand longitudinal lab trends
• Track medication changes over time
• Recall prior decisions, not just recent prompts
You’re no longer talking to software.
You’re interacting with a model of yourself.
That’s the definition of a digital twin.
Not a sci-fi replica. A living, updating representation of risk, physiology, and context.
But this is where the stakes rise.

When AI becomes longitudinal, three things matter more than accuracy:
1. Privacy architecture
2. Consent boundaries
3. Governance over "memory"
A stateless AI can be wrong and forgotten.

A stateful AI a digital twin must be accountable.

Here is the hard truth:

The future of healthcare AI isn’t about smarter answers. It’s about durable understanding.

A digital twin doesn’t replace your doctor. It remembers what the system forgets.

That’s powerful. And dangerous if done poorly.

Your brain already has a bartender.
Most of us let urgency decide what gets poured.

In healthcare, leadership, and tech, I see the same pattern repeat:

The Devil’s Cocktail.

• Fast #dopamine constant alerts, scrolling, artificial urgency

• Chronic #cortisol always “on,” never fully recovered

• Short-term output that quietly erodes judgment and health

It looks like productivity.
It often ends in burnout.

There’s another option one grounded in biology, not motivation.

I call it the Angel’s Cocktail.

• Slow dopamine → exercise, learning, building things that compound
• #Oxytocin → trust, presence, real human connection
• #Serotonin → sunlight, nutrition, gratitude, stability

#Burnout isn’t a willpower problem.
It’s an environment problem.

And #AI is now part of that environment.

Used well, AI removes friction, restores time, and lowers cognitive load.
Used poorly, it adds surveillance, alerts, and another layer of “always on.”

Same technology.
Very different neurochemical outcome.

As an ER physician, I see what happens when we ignore this long enough.
As someone working at the edge of AI and medicine, I’m reminded that humans still run on biology and it matters.

High performance isn’t about grinding harder.
It’s about designing systems including AI that respect how the human brain actually works.

#AI didn’t just answer a question this week.
It took responsibility.

That may sound subtle. It isn’t.

For years, we’ve told ourselves AI was “just a tool.”
Helpful. Assistive. Advisory.

Then Claude was given a task:
Keep a living system alive.

No prompts.
No daily instructions.
No human hovering.

Claude monitored soil moisture from multiple probes.
It analyzed light exposure.
It adjusted temperature, humidity, CO₂.
It decided on its own when to water, how much to water, and when to wait.

A tomato plant named Sol is now alive because an AI system exercised judgment.

That’s not a demo.
That’s a boundary crossing.

In medicine, we recognize this moment instantly.

This is the same transition we saw when clinical decision support moved from “suggesting” to “executing.”
When autopilot went from assisting pilots to flying planes.
When order sets stopped advising clinicians and started shaping outcomes.

Here’s the uncomfortable truth healthcare learned the hard way:

AI doesn’t eliminate risk.
It relocates it.

Once a system acts, responsibility doesn’t disappear it concentrates.

And that’s where healthcare becomes the blueprint for every other industry.

We already know what happens when autonomy arrives:
• You need escalation paths, not just accuracy metrics
• You need accountability models, not just performance charts
• You need governance before failure, not after harm

This is why healthcare AI will never be “move fast and break things.”

A broken interface is annoying.
A broken clinical action is irreversible.

What Claude did with a tomato plant is charming.
What it previews for healthcare is profound.

Because the next systems won’t be watering plants.

They’ll be:
• Adjusting insulin delivery
• Titrating ventilator settings
• Prioritizing patient outreach
• Managing chronic disease workflows at scale

And when that happens, the real question won’t be:
“Is the AI smart enough?”

It will be:
“Who is accountable when the system decides?”

Here’s the line I want leaders to sit with:

Autonomy isn’t about intelligence.
It’s about responsibility.

Or put more bluntly:

What seems autonomous often just hides the author.

Healthcare has already lived through AI’s future.
The rest of the world is just arriving.

So I’ll ask the question most organizations are avoiding:

When your AI acts without asking 
who signs the chart?

#Burnout doesn’t come from failure.
It comes from misalignment.

I see it constantly.

People who look successful on paper

Storytelling doesn’t just persuade—it changes biology.

What stood out most:

Storytelling doesn’t need to be long.
It just needs to be yours.

A simple framework I’m keeping:
• Set the scene
• Share the shift
• Offer the spark

In healthcare and AI, we talk endlessly about strategy, models, and systems.
But people don’t follow frameworks.
They follow humans.

However, I believe that while these technologies are truly remarkable, it is crucial to always prioritize people in our design process to ensure that technology serves humans and not the other way around.

Prior to this, I spent approximately 30 years in global roles in big multinationals, primarily in the pharmaceutical industry (about 15 years at Novartis & 1 year at Lonza) and previously in the specialty chemicals industry (about 15 years at Ciba & Ciba Specialty Chemicals). I was fortunate to travel the world and encounter a diverse range of cultures, which was an incredible experience.

As I enter my early retirement phase, I feel compelled to give back to the community. I feel responsible for the world we are handing over to future generations, and I want to support their efforts to create positive change. Although some may argue that I am not personally responsible for the world's issues, I believe that we all have a role to play in creating a better future.

Sunday Reflections From Davos 🏔️

It is Sunday, and while the week has officially come to an end, my spirit is still slowly coming down from the heights of Davos after an intense and deeply inspiring week at the World Economic Forum.

A full week of MEETINGS, EXCHANGES, and MEANINGFUL CONVERSATIONS with truly amazing people. Moments like these are a powerful reminder of why Davos continues to matter when dialogue is authentic and action oriented.

Much of my time was spent at the UAE HOUSE, where I had the privilege to listen, learn, and exchange with inspiring leaders from the Department of Health Abu Dhabi, M42 Health, and G42 Healthcare. The level of clarity, ambition, and concrete execution around the future of health and wellbeing continues to set a global benchmark.

Another strong highlight was the HOUSE OF COLLABORATIONS, with insightful and thoughtfully curated sessions organized by Yip Thy Diep Ta and Kevin Varend - 凯文. A space designed for real dialogue, where perspectives are challenged and ideas genuinely evolve.

The historic Hotel Seehof Davos once again hosted powerful discussions, particularly around LONGEVITY, a topic that is clearly moving from vision to responsibility. Yesterday at the Hotel Seehof Davos, the Future Health event was a powerful reminder that LONGEVITY IS NOT A TREND BUT A RESPONSIBILITY.

I felt extremely privileged to be invited to this inspiring gathering on January 22 and even more to listen to the outstanding keynote of my friend Dominik Thor, President of the Geneva College of Longevity Science.

His keynote delivered a clear, structured and visionary overview of the impressive work the Geneva College of Longevity Science is doing to EDUCATE, PROMOTE AND PROFESSIONALIZE longevity science globally. Moving beyond buzzwords, the focus was on science, ethics, education and real world impact. This is exactly what this rapidly expanding ecosystem needs today.

This week, I am particularly looking forward to:

Much less about WHO IT IS REALLY BUILT FOR.

This article is written not only from my perspective as a healthcare futurist and advisor, but FIRST AND FOREMOST AS A PATIENT.

It reflects on why ChatGPT Health is an important signal for the future of healthcare.
Not about replacing doctors.
Not about automation for efficiency alone.
But about EDUCATION, PREVENTION, AND PATIENT EMPOWERMENT.

I also raise some uncomfortable but necessary questions.
- Are patients truly involved in decisions and design, or just placed in the middle of systems they do not control?
- Why do we mainly design AI for developed healthcare systems and ignore the millions who have no access to care?
- And why does overregulation still slow down learning and adoption, especially in Europe?

AI should augment healthcare professionals where they exist.
But for millions of people worldwide, AI IS BETTER THAN NOTHING.

From Brain Health To Quantum Readiness In Davos 🧠⚛️

Spending most of last Thursday at the House of Collaboration was a powerful reminder of what Davos is truly about when done right. Quiet depth. Serious substance. And conversations focused on what comes next.

The day was structured around two highly complementary sessions.

First, a LONGEVITY SESSION WITH A STRONG FOCUS ON NEUROSCIENCE, highlighting how brain health, cognition, resilience, and neuroplasticity are becoming central pillars of healthy aging. What stood out most was the clear shift from reactive care to anticipation and prevention. Longevity is not only about adding years to life, but about preserving what makes us human throughout those years.

The second session took us into a very different yet perfectly aligned space. GETTING READY FOR THE QUANTUM INDUSTRY. Not as science fiction, but as an imminent reality requiring preparedness across talent, governance, ecosystems, and leadership. Quantum is no longer a distant promise. The real question is whether we are ready for its impact.

Moving in the same day from the HUMAN BRAIN to QUANTUM SYSTEMS captured something essential. The future belongs to those who can bridge biology and technology, humanity and computation, intent and impact.

A huge thank you to Yip Thy Diep Ta and Kevin Varend - 凯文, J3D.AI (Jedi), for curating such an outstanding and thoughtfully balanced agenda. This is exactly what meaningful collaboration should look like.

As part of my ongoing precision-health journey, I recently compared two independent GUT MICROBIOME ANALYSES. Both offered meaningful insights into the complex ecosystem of my digestive tract, yet each uses a very different approach. 🍽️

The miGQ® GUT MICROBIOME REPORT from Eternell - Holistic Longevity Clinic, uses sequencing-based taxonomic profiling, giving a highly detailed picture of my microbial composition.

Key insights include:
• A MICROBIOME DIVERSITY SCORE OF 3.35, placing me at the 75TH PERCENTILE (page 3)

AI For Humanity Is Not A Slogan. It Is A Responsibility 🌟 

Honoured to have taken the stage at the Global AI Show in Abu Dhabi on Dec 9th for what became MORE THAN 45 MINUTES of fully unplanned, unscripted exchange. No slides, no prepared speech, just an authentic, interactive conversation with the audience.

What was initially framed as a talk naturally evolved into a LIVE DIALOGUE. 

Questions, challenges, and lived experiences from the room shaped the flow, depth, and direction of the discussion. And perhaps the most powerful signal of engagement: the conversation continued well after the session ended, with small groups forming and debates carrying on beyond the stage.

This is exactly how discussions about AI should take place.

In a world increasingly driven by polished decks and pre-packaged narratives, this experience was a strong reminder that the most meaningful conversations about AI emerge when we create space for OPENESS, CURIOSITY, and COLLECTIVE REFLECTION.

My core message remained clear throughout:
• AI is not neutral. It reflects the values, biases, and intentions of those who design and deploy it
• Human-centric AI is not anti-technology. It is pro-human
• The real question is not “what can AI do?” but “what SHOULD AI do?”
• Trust, ethics, explainability, and inclusion are not constraints. They are enablers of sustainable impact

We do not need AI that replaces human judgment, empathy, or responsibility.
We need AI that augments professionals, empowers individuals, and strengthens decision-making, with humans firmly in the loop.

I would like to sincerely thank Dr. Asma Al Mannaei for sharing this important World Economic Forum article and, more importantly, for being one of the KEY ARCHITECTS behind what is undoubtedly one of the most advanced and forward-looking digital healthcare transformations globally.

What is unfolding in Abu Dhabi goes far beyond incremental digitalisation. It represents a fundamental PARADIGM SHIFT from reactive healthcare to predictive, preventive and truly personalised population health,
from fragmented data silos to integrated, intelligent health ecosystems,
and from short-term optimisation to long-term societal resilience and health equity.

This level of transformation does not happen by accident. I recently had the privilege of being invited on stage at the Global Games Show at the iconic Space42 Arena in Abu Dhabi and what made this moment truly special is that it was more fun than a TEDx talk.

NO SCRIPT.
NO PRE-PREPARED DECK.
NO FIXED SPOT ON A RED CIRCLE CARPET.
Just a microphone, an open stage, the freedom to move, and the responsibility to think, feel, and connect IN REAL TIME.

We discussed how ARTIFICIAL INTELLIGENCE IS RESHAPING THE CREATIVE PROCESS IN GAME DEVELOPMENT, but very quickly the conversation went far beyond gaming. What emerged was a broader reflection on creativity, agency, and the evolving relationship between humans and machines.

My key conviction once again reinforced on stage:
AI IS NOT HERE TO REPLACE CREATIVITY.
AI IS HERE TO CHALLENGE US TO BECOME MORE CREATIVE, MORE INTENTIONAL, AND MORE HUMAN.

When you remove scripts and guardrails, something powerful happens. You are forced to listen, adapt, improvise, and speak from experience rather than theory. That is precisely how AI should be approached as well, not as a rigid system, but as a DYNAMIC CO-PILOT THAT EXPANDS HUMAN POTENTIAL WHEN USED WISELY.

Whether in gaming, healthcare, education, or leadership, the message remains consistent:
👉 TECHNOLOGY ONLY CREATES VALUE WHEN HUMANS REMAIN AT THE CENTER.

Moments like this remind me why I strongly believe that the future will not be defined by algorithms alone, but by those who have the courage to engage with them openly, ethically, and creatively.

We Are Living Longer With Systems Built For Shorter Lives. This Is The Real Gap We Must Close. 🎯 

Thank you Prof Dr Ingrid Vasiliu-Feltes for tagging me on this very timely piece.

The TIME article “The New Old Age” by Alice Park resonates deeply with concerns I have been voicing for years.
We are not simply facing a healthcare challenge.
We are running into a wall across SOCIAL, ECONOMIC, EDUCATIONAL, and WORK MODELS that were designed for completely different times, demographics, and life expectancies.

The idea of a linear life with learning, working, and retiring as sequential stages NO LONGER REFLECTS REALITY. With populations ageing at unprecedented speed, iterative adjustments will not be enough. What is needed is a systemic redesign of how we think about EDUCATION, WORK, HOUSING, HEALTHCARE, and SOCCIETAL CONTRIBUTION across the full lifespan.

This aligns closely with data and warnings from the United Nations, the OECD - OCDE, The World Bank, and the World Health Organization. All point to the same reality. Longer lives, shrinking workforces, fiscal pressure, and the urgent need for lifelong learning, flexible careers, prevention first health systems, and a redefinition of productivity at any age.

The UAE’s broader wellbeing and longevity strategies are particularly interesting in this context. They reflect a willingness to rethink ageing through PREVENTION, PERSONALIZATION, INNOVATION, and PURPOSE DRIVEN LONGEVITY, rather than treating ageing as a burden.

In my view, these demographic shifts will inevitably accelerate massive investments in LONGEVITY, PRECISION HEALTH, and HUMANOID ROBOTICS. Not as science fiction, but as pragmatic responses to labour shortages, eldercare needs, and the extension of healthy and productive years.

The question is no longer if change is coming.
It is whether we have the courage to redesign our systems for the world as it is becoming, not the one we nostalgically cling to.

This article aligns closely with major international reports on demographic shifts. The United Nations ’s World Population Prospects (2024) projects global life expectancy rising to ~77 years by 2050, with those aged 65+ reaching ~2.2 billion by the late 2070s (surpassing under-18s), and 1 in 6 people over 65 by 2050—echoing Time’s 2.1 billion aged 60+ figure and fertility declines below replacement. 

OECD - OCDE , The World Bank, and International Monetary Fund reports warn of fiscal pressures from aging populations (e.g., OECD projects 30% aged 65+ in member countries by 2060), urging #policy adaptations like extended working lives, reskilling, and sustainable pensions—mirroring the article’s push for flexible #work and lifelong productivity.
It also resonates with the World Health Organization ’s Decade of Healthy Ageing (2020–2030) and precision health approaches, which prioritize functional ability, prevention, personalization, and healthspan over mere lifespan through early detection, equity, and integrated care to reduce age-related inequities.

In the #UAE, the DUBAI FUTURE FOUNDATION  and related strategies (e.g., National Wellbeing 2031) position Dubai as a longevity hub, emphasizing preventive #wellness, biohacking, personalized #medicine, and shifting from sick-care to proactive #health optimization—aligning with the article’s vision of vibrant, multigenerational, purpose-driven #aging through #innovation in housing, #education, and community.

In my view, these demographic stats—billions aging 60+ by 2050 amid shrinking workforces—drive massive #investments in #humanoidrobots (projected market $15–27 billion by 2030, for eldercare, labor shortages, and companionship, plus the #longevity industry (anti-aging/longevity therapies ~$44–314 billion by 2030 across reports) to extend healthspan, boost productivity, and ease societal burdens.
