Early in my career, I sat in a boardroom thick with pressure.

A nurse once told me : 
"We're trained to reduce a Black patient's pain score by 2 and increase an Asian patient's by 2."

The assumption was that Black patients exaggerate their pain, and Asian patients tend to tough it out.

She said it matter-of-factly. I sat with it for a long time.

If that kind of thinking is baked into medical training, it's going to end up baked into the data. And if it's in the data, it'll end up in the AI.

A 2025 scoping review of 8 studies (22,259 patients) found racial/ethnic disparities in postpartum pain: Black and Hispanic patients reported higher pain scores yet received less frequent assessments and lower analgesic doses compared to White patients.

This doesn't come from nowhere. 

A 2016 PNAS study found that half of white medical students (in the sample) held false beliefs about Black patients, including the belief that Black people have a higher pain tolerance. Those who held these beliefs tended to underestimate Black patients' pain and made less accurate treatment recommendations.

This myth traces back to J. Marion Sims, who performed surgical experiments on enslaved Black women in the 1840s without anesthesia, claiming that they felt pain differently. 

That ideology never fully left medicine. 

But the consequences go beyond the hospital room.

Undertreated labor pain increases psychological trauma, raises postpartum PTSD risk, and erodes trust in the healthcare system. It drives Black mothers away from the very institutions that are supposed to track their outcomes. 

Which means less documentation. Less treatment in the record. Less signal in the data.

When clinical AI tools learn from EHR data, they inherit this pattern. If your product uses historical pain data to benchmark risk, recommend interventions, or personalize care, you may be training on a record of what was documented, not what was actually experienced.

LinkedIn Strategy for Women’s Health Founders & Advisors | Turning Your Expertise Into Content That Attracts Investors & Partners | Ex-Pharma | Coffee Addict

This week, more than 200 women are pushing for Canada to adopt a proven national framework for women's health.

Bill S-243 is establishing Canada's first national framework for women's health.
It is modelled on the National Framework for Diabetes Act published 2022.

The diabetes framework was designed to align efforts across provinces, reduce duplication, and provide coordinated policy direction where systems were fragmented.

Now Senator Hon. Danièle Henkel is applying that same legislative approach to women's health.

S-243 targets four key areas:

Research and innovation
Primary care and prevention
Healthcare worker training
Access for underserved women (rural, Indigenous, racialized, 2SLGBTQI+)

This week (Feb. 25–26), 200+ women's health advocates gathered on Parliament Hill to push it forward, bringing evidence, expertise, and lived experience to engage MPs and Senators.

They know what founders and clinicians know: 
Fragmented systems create gaps in care and inefficient use of [limited] resources.

Bill S-243 is designed to clarify where money flows, identify gaps, and promote care coordination.

In the 1850s, a slaveholder used spirometry to claim that Black lungs were deficient and that forced labour was the cure. Centuries later, that logic is still embedded in modern diagnostic tools.

Most people don't know that spirometry (the standard test for diagnosing asthma, COPD, and other lung conditions) has long applied automatic "race corrections," raising the threshold at which a Black patient's lung function is flagged as abnormal.

This is based on the assumption that Black patients have 10–15% less lung capacity than white patients. 

The correction isn't just a number adjustment. It changes who gets diagnosed and who qualifies for treatment. 

And when lung disease goes undetected, the consequences compound: undertreated asthma and COPD worsen pregnancy complications and increase respiratory failure risk.

None of this has a genetic basis. 

The differences in measured lung function are better explained by poverty, environmental exposures, and occupational hazards, factors that were systematically excluded from the original research  

Yet the race correction was still formalized into the NHANES III equations embedded in spirometry software worldwide.

The costs to patients is real

- In one study of 14,080 pulmonary function tests for Black patients, removing the race correction led to an additional 665 patients diagnosed with restrictive disease.

- A Detroit study found that race-corrected spirometry hid lung function issues in up to 1 in 5 Black children with confirmed asthma symptoms.

The American Thoracic Society issued race-neutral guidelines in 2023, but adoption is uneven, and no major genetic locus has been found that justifies the original correction in the first place.
