Nearly 75% of Medicaid beneficiaries receive services through Managed Care Organizations (MCOs), with over half of all Medicaid dollars flowing through capitation contracts with plans such as Aetna, United, Centene, Molina, and Elevance.

However, when Medicaid pilots fail, the blame often falls on:
• The technology
• The vendor
• The adoption curve

In truth, many failures stem from issues related to procurement and MCO contracting.

In the coming week, I will explore:
• Why pilots often become stuck in “contracting purgatory”
• How MCO incentives can hinder scaling efforts
• What true procurement modernization should entail

It is essential to recognize that scaling 21st-century care models requires moving beyond 20th-century procurement practices. Additionally, modernizing Medicaid is impossible without rethinking how innovation is acquired.
