The Most Radical Leadership Model in Healthcare Didn’t Come from Business-It Came from Mr. Rogers.

Fred Rogers did something most leaders never learn to do. He made people feel safe enough to think.

He didn’t teach by authority.
He taught by regulation.

That matters more in healthcare than almost anywhere else.

Before patients understand information, they scan for safety.
Before clinicians absorb strategy, they assess emotional threat.
Before teams perform well, they need regulation, not motivation.

Mr. Rogers never talked down.
He translated complexity without stripping dignity.
He explained hard things slowly, clearly, and without ego.

Mr. Rogers moved slowly on purpose.
Slowness creates room for people to show up as humans, not just performers.

He also separated worth from productivity. You mattered because you existed, not because you produced.

In a system obsessed with RVUs, throughput, and metrics, that distinction is quietly radical.
Burnout is not always about workload.
Often, it is about identity collapse.

And perhaps most importantly, he modeled presence over performance.
There was no gap between his values and his behavior.
No leadership persona.
No mask.

Mr. Rogers understood something we keep relearning the hard way.

Feelings are not a distraction from the work.
They are part of the work.

The leaders who understand that are not soft.
They are precise.

That is a neighborhood worth building.

Watching Adrienne Towsen, MD  build her next chapter has been a masterclass in quiet confidence.

She is launching a new orthopedic practice while competing as a dancer on the world stage, and instead of hiding the transition, she is documenting it with clarity, grace, and intention. No performative hustle. No overexplaining. Just disciplined work, creative expression, and self-trust.

Surgeons are about to find out how long “normal” can actually operate.

The current orthopedic model was never designed for this level of administrative drag, reimbursement compression, and emotional exhaustion.

It survives by volume.
And volume is becoming harder to maintain.

We talk about burnout as if it’s a personal failing.

It’s not.

It’s a system running past its design limits.

That is why ideas that once felt taboo are now being discussed openly.

Concierge care.
Cash components.
Hybrid models.

Not because surgeons suddenly want something different.

But because the existing structure is no longer viable for many practices.

These models will be unpopular.
They will be criticized as elitist.
They will be framed as abandoning patients.

But the uncomfortable truth is this.

When a system stops working, people build workarounds.

This isn’t ideology.

What’s interesting is not the criticism.

The ones criticizing today will be forced to redesign tomorrow.

This isn’t about being right early.
It’s about being honest before the math makes the decision for you.

Riding for a local Northwest Arkansas brand this weekend.

I’ll post something on LinkedIn that gets 250 or 300 impressions.


No spike.
No dopamine hit.
No algorithmic fireworks.

And yet, those posts quietly generate the biggest ROI of anything I write.

Here’s why.

When a post is hyper-niche, it is not designed to attract everyone.
It is designed to repel almost everyone.

The only people who click are the left-handed, one-eyebrowed orthopedic and Neurosurgeons who are done with KOL politics, interested in building trust (at scale)m want to monetize their reputation, and are thinking seriously about becoming a (DOL) Digital Opinion Leader. {MY TRIBE}

That is not a metaphor.
Those are the people who end up calling me.

LinkedIn has a funny way of questioning your identity when you do this.
It asks why you’re posting.
It tempts you to chase likes.
It nudges you toward safer, broader, more agreeable content.

And to be clear, there is nothing wrong with writing for reach.
Likes matter.
Visibility matters.

But thought leadership is a different game.

Thought leadership is about precision.
It is about speaking so specifically that the right people feel seen and everyone else scrolls past.

Riches don't have to come from the masses. Read Kevin Kelley 1000 True Fans. 

[They come from the niche that recognizes itself in your words.]

If you are writing content that feels almost too specific to post, you are probably closer than you think.

Low impressions do not mean low impact.
They often mean the opposite.

Most people build alone.


They write the book solo. Launch the course by themselves. Start the podcast with no co-host.

And most of them quit.

Not because the idea was bad. Because the weight of doing everything alone eventually breaks you.

Here’s what I’ve learned after:

- releasing Physician Brand Rx with Scott Sigman MD 
- co-building three $7-figure business units with talented friends.  

1+1=3.

Two people committed to the same creative project don’t just split the work. They multiply the output.

You show up on the days your partner can’t. They show up on yours. The accountability alone is worth the “cost” of sharing credit.

But it goes deeper than that.

Your network plus their network. Your skills plus their skills. Your blind spots covered by their strengths.

The book you’ve been “meaning to write” for five years? You’d finish it in six months with the right partner.

The course sitting in your head? It would actually exist.

I discuss how physicians can navigate LinkedIn's algorithm by shifting focus from attention-seeking to building genuine authority. 

We talk about three essential steps: 

1. developing consistent digital thought leadership that showcases your unique perspective.

2. establishing authorship through branded and proprietary protocols that become synonymous with your name.

3. leveraging proactive outreach to connect with decision-makers by sharing ideas rather than selling. 

I explain how this approach helps surgeons align with like-minded partnerships and monetize their reputation authentically.

This is me talking to the version of Matthew who is tempted to explain instead of insist.

He would warn you against explaining yourself too much.

The more you try to make your work legible to everyone, the more you collapse the tension that makes it valuable.

 -If everyone agrees with it, it is not leadership.

He would specifically caution you about drifting into marketing language.

He would also warn you about metrics theater.
 Impressions.
 Engagement.
 Follower growth.

He would remind you that premium buyers do not buy dashboards.
 They buy relief.
 Clarity.
 A feeling of being seen and positioned correctly.

Finally, he would warn you against borrowing authority from surgeons instead of transferring it back to them.

If your voice ever becomes louder than theirs, you become the brand.
And premium clients do not want to rent someone else’s identity.
They want to author their own.

The Case for Thinking “Minimally Traumatic” in Knee Replacement.



Minimally invasive is a phrase we’ve all used.

It sounds reassuring.
It sounds precise.
And it sounds like progress.

The problem is that it describes the operation, not the experience.

Over time, I’ve found that what patients struggle with after knee replacement has very little to do with the incision.

They struggle with swelling.

With inflammation.
With pain that spikes unpredictably.
With the first two weeks when everything feels uncertain.

That’s why I’ve moved away from thinking in terms of minimally invasive and toward minimally traumatic.

Minimally traumatic isn’t a technique.

It’s a philosophy.

It forces you to think beyond how you get into the knee and focus on what the knee has to tolerate afterward.

-How much physiologic stress you create.
-How consistent the early recovery feels across patients.
-How much volatility shows up in those first days when confidence is fragile.

You can have a small incision and still create a very traumatic recovery.

You can also be disciplined, deliberate, and thoughtful in a way that makes the entire experience more manageable for the patient.

Language matters because it shapes expectations.

Expectations shape how recovery is experienced.

And recovery is what patients live with long after the operation is over.

For me, minimally traumatic is a better standard. 

Not because it sounds better.

But because it more accurately describes the outcome surgeons are responsible for.

I spend a lot of time helping surgeons give precise language to standards they already practice but have never formally named.

The work is rarely about changing behavior.

It is about removing ambiguity so the right expectations can form.

Coming off of active duty in the SEAL Teams…

Or, at least, I had never heard of any of them at the time.

Mind you, this was “back in the day…”



Until recently, this was also true for college athletes, nationally-ranked athletes, Olympic athletes, and professional athletes.

10 p.m. (2200)...  Day one complete as soon as I get down another half gallon of water (no flavor) between now and when I go to bed...


Got a little behind in my water today, but commitment is a commitment.


Yes, it is the second day of the year, but Reggie Ordonez made the second day of the year the first day of “77 STRONG.”

There is still time to jump in!

This is not a “New Year’s Resolution…”



This is 77 days of executing these 5 simple tasks:

45-minute workout
45 minutes of scripture and prayer
One gallon of water (no flavor)
Follow a diet. No cheats. No alcohol.
10 pages of nonfiction



Yes, I know there are a bunch of people on here that do way more than this every day, without 77 STRONG, but this resonated with me, strongly (no pun intended), this year.



No, I am not going to give you the daily update… 

No one wants to endure that.

I may share some updates on occasion when the Holy Spirit moves me.



Reggie’s 77 STRONG caused me to put more structure and measurement to these important tasks.

This also caused me to start some things that I had thought about in the past, but never executed…  Hmmm…



The first couple of things that shouted out to me were that I don’t drink enough water...

That was never a habit.

I am feeling the pain of not putting enough planning around that one this evening…



The other thing that I had thought about doing, but had not started until this year (yesterday, quick head start…), is Father Mike Schmitz’ “Bible in a Year” from Ascension.

I am also going through John Cannon's Monk Mindset challenge, as well, as part of 77 STRONG.



As for workouts, they are much more planned and structured this year.



As for the no alcohol, that is probably the easiest one for me as I celebrated 10 years of sobriety two months ago.



As for the rest of the diet commitment, I hate to admit that I have been known to drink soda…

No more soda. Yes, I know, I should not have been drinking it anyway…



Lastly, as for the 10 pages of nonfiction, I am in a small book club and we are reading Gino Wickman's book, TRACTION.

When parents meet who continue to endure the loss of their children to suicide, a significant bond is formed… forever… and…



Amazing things can still happen.



Here is the short version…

The first note I sent to Leslie Weirich via LinkedIn DM was in November of 2021.

That was two years and 7 months after we lost our Leslie, aka “Lulu,” to suicide.

Lulu was 18.



Leslie and I finally met face-to-face about a year and a half later, in May of 2023, when she invited us to a fundraiser where she was speaking on behalf of a full-length feature film being produced with some ties to suicide, “Adieu.”



There, we met the writer/director, James Meyer, and his bride who is also the producer, Sara Meyer, both of Lava Lamp Films.



Six months later, in November of 2023, I was able to have breakfast with James and personally thank him for the unbelievable portrait he had drawn of Lulu and sent me from Europe (the portrait, itself, and the details of which are in a previous post).



In the meantime, at that fundraiser in May of 2023, Molly Stamer and her son, Jake Stamer, were at our table, as well (thanks to Leslie), and we had an enjoyable time.



Roll tape forward two and a half years later, this fine young U.S. Marine, Jake, reached out to me in November of 2025, cordially, with a specific reference to Lulu’s birthday…

Now, not as many people remember Lulu’s birthday as much these days, and he was quite a gentleman to mention it.



Well, we finally grabbed some time last week and discussed his goals and ambitions for life, and we enjoyed a meaningful, authentic, and deep conversation.

It was kind of a two-for for Jake as one of the great mentors on the planet, Rick Schultz, was there, as well.



Leslie, thank you for improving all of our lives with you in it.

James and Sara, keep going! Your work is important!

Molly, you have awesome sons. I know you have two, but I have only met Jake. Nice work!



It is a small world.

Let’s keep making it smaller.

Well, LinkedIn busted us…



Matthew Ray Scott has not only instrumental helping me build three of my prior companies, but we have also become close “later in life” dear friends since 2014.



Matthew is a top physician brand strategist and founder of "FEED. The Agency," an award-winning healthcare marketing agency. 

He launched FEED in 2010 and grew it into an 8-figure company, working alongside his mother, wife, daughters, and even best friends. 

His work helps surgeons and healthcare innovators stand out from the "sea of sameness".

Matthew is also an accomplished author.



What truly sets Matthew apart is his heart-led leadership and values. 

As a young Army officer, he made the courageous choice to leave the service after coming home from a deployment and realizing his baby daughter had taken her first steps without him. 

That speaks volumes about his priorities. 

He carries that same people-first mindset into his work.

As his Army service is mentioned here, books could be written about what he did in the service. 

I will just leave you a summation that his service in special operations was more than noteworthy in several extremely hazardous missions in foreign countries throughout the world.

Eighteen (18) men got together this morning in a Men’s Spirituality Group (MSG).  So what?


Well, these Roman Catholic men have been getting together on the second Saturday of every month for the last 17 years…

Rain or shine.  Blizzard or heat wave.

Sometimes it is 18 to 23 of them...

Sometimes it is 12...



They pray.

They share some of life’s deepest and darkest trials and tribulations…

They share family achievement, successes, and hardships…



It is not a therapy session.



It is a way men actually are there for one another.

It is a way that men help one another be the best husbands, fathers, friends and professionals they can be.

There is no place like this.



They read scripture.

Most Saturdays, a priest joins them.

On the Saturdays the priest is cannot make it, they listen to Bishop Barron’s reflections on the Sunday gospel.



They pray for intentions for one another.

They pray, again.



They come back the next month on the second Saturday…



I am grateful for these Saturdays from 7:30 a.m. to 9:00 a.m. at Our Lady of Perpetual Help Catholic Church in Glenview, IL.

I don’t make it every second Saturday, but I make the ones I can.

These are great men.



Sometimes you see them “out in town,” too!

Bennett Weiss is one of these men (pictured).

It was great to see him “in the wild” at a recent conference.



I remain grateful for William Harlan who founded MSG (and still runs it), and for Rick Schultz who introduced me to MSG.

We are all better men for it.



Find your tribe and stay with them.

Not just for yourself, but for them, too.

Yes, our Dad said something very similar to us growing up, but these are the words from the late, J. Pierpont Morgan.



Those words capture the essence of J.P. Morgan and the mindset that defined his greatness.



As a former teammate in what is now J.P. Morgan Private Bank, “back in the day…”, we learned that Morgan did not become one of the most influential figures in American history simply by inheriting opportunity. 

He earned it through discipline, conviction, and an unrelenting work ethic. 

He believed deeply in choosing a path aligned with one’s strengths and passions, and then committing to it fully. 



As we studied, we learned for Mr. Morgan:
Effort was non-negotiable. 
Vision without execution meant nothing.



He also understood that real power is built on trust.

In an era of financial chaos, Morgan’s reputation for integrity allowed him to stabilize markets, reorganize industries, and restore confidence when few others could. 

His leadership during crises reminds us that calm, principled action matters most when fear is loudest.

Even in downturns, he believed progress would prevail. 

That optimism, paired with hard work, enabled him to help shape modern American industry and finance.



The lesson is timeless: 

Follow your own path.
Work relentlessly.
Act with integrity.
Believe in what lies ahead. 



Greatness is rarely accidental.
