🗣️ People often confuse purpose with profession.
But they are not the same.

Profession is what we become skilled at.
Purpose is what we are willing to carry, even when there is no guarantee of recognition, reward, or even visible credit.

I understood this more deeply when I was working on a fertility startup.

We spent a lot of time between two worlds:
patients and IVF experts.

What struck me was not a lack of intent.
Both sides wanted the same thing: a successful outcome.

But they defined value very differently.

When I spoke to IVF experts, they naturally viewed success through a professional lens.
The most meaningful cases, for them, were the ones where deep domain expertise made the decisive difference.
The more complex the intervention, the more visible their contribution felt.

But patients did not see the journey that way.

They were not measuring the depth of specialization involved.
They were measuring something much simpler, and much more human:

Who gave me the result?

That is where the disconnect lived.

To the expert, contribution mattered.
To the patient, outcome mattered.

And then there was another layer.

Despite standing on opposite sides of the experience, both patients and doctors leaned on the same decision-making shortcut:

external validation.

Doctors respected referrals, peer reputation, and professional credibility.
Patients trusted word of mouth, recommendations, and social proof.

Different lenses.
Same dependence on validation.

That experience stayed with me because it revealed something larger than healthcare.

In many fields, we assume people value what we value.
They rarely do.

Experts often want their depth to be recognized.
People often remember only the result that changed their life.

And perhaps the hardest part is this:

The person who changed the outcome may not always be the person who receives the most recognition.
The expert remembers the complexity of the intervention.
The patient remembers the moment hope turned into relief.

Maybe purpose begins exactly there —
in continuing to serve, even when the story will be told in someone else’s name.

Medication adherence has long been constrained by binary instruments and backward-looking metrics. These tools are effective at recording whether behavior occurred, but they rarely explain why adherence erodes, why it collapses suddenly, or why interventions succeed in some patients and fail in others. Human behavior is not static. It fluctuates under stress, reorganizes in response to context, and carries structural vulnerabilities that are invisible when measurement is limited to retrospective scoring. Measuring adherence without understanding its stability over time leaves healthcare systems reacting to failure rather than anticipating it.

My work builds on a strong foundation. My father, Dr. Donald Morisky, developed the Morisky Medication Adherence Scale, creating a global standard for identifying non-adherence with reliability and clinical relevance. That contribution gave the field a shared language for detecting risk. I honor that legacy by extending it forward. The MMAS tells us when non-adherence is present. The unanswered question is why adherence persists in some systems while it destabilizes in others, even when risk is already known.

The Theory of Predictive Emergence reframes adherence as a dynamic system rather than a static outcome. Behavior is treated as a position within a three-dimensional state space, defined by three interacting domains of system integrity. Instead of summarizing the past with a score, the framework evaluates how aligned the system is in the present and how that alignment constrains what is likely to occur next.

Adherence is not labeled high or low. It is represented as an XYZ coordinate in behavioral state space. As that coordinate shifts, movement becomes observable. Drift, instability, and recovery appear as trajectories rather than isolated events. Change is no longer inferred after failure occurs. It is visible as motion through space.

The geometry matters because the domains interact multiplicatively, not additively. Strength in one area cannot compensate for weakness in another. A system can appear functional while already being structurally unstable. When misalignment crosses a threshold, failure is not gradual. It is abrupt. Predictive Emergence is designed to identify that instability before it manifests as missed doses, treatment discontinuation, or downstream clinical harm.

Prediction in this framework is not an overlay applied after measurement. It is an emergent property of system structure. When behavior occupies certain regions of state space, its future becomes constrained by geometry itself.
