There is a moment in almost every skill where things start to feel easier.

Not because the work became simple.

Because you have done it enough times that your brain recognizes the patterns.

That moment happened to me this week.

I was reviewing an application for someone and preparing feedback. As I worked through it, the adjustments that were needed felt obvious. The gaps. 

The places where the story could be clearer. The pieces that needed to connect.

The suggestions just rolled off the tongue.

It reminded me how much practice sits behind moments like that.

Years of reading applications. Writing them. Seeing what works. Seeing what does not.

Practice is what creates that shift.

The more time you spend learning the fundamentals, the more room you have later to experiment, adapt, and think creatively.

It applies to almost everything. Leadership. Strategy. Writing. Problem solving.

The early reps may feel repetitive.

They are also what make the interesting work possible later.

After two decades operating at the highest levels of Western medicine, I remain a strong believer in what modern medicine can do.

If you suffer a traumatic injury, a life-threatening condition, or a structural problem that requires surgery, our system can save lives. I see it every day.

But when it comes to chronic disease, we are falling short.

I reached a breaking point watching patients in their early 40s come into my clinic for spine surgery who were already obese, diabetic, and taking multiple medications. Surgery could address their spine. But it could not address the underlying biology that created the problem in the first place.

This is the uncomfortable reality we need to confront.

Fewer than 10 percent of adults today are considered metabolically healthy. At the same time, more than 129 million Americans are living with chronic diseases driven largely by lifestyle, inflammation, and metabolic dysfunction.

Our current system excels at treating acute problems. It is far less effective at preventing or reversing the biological processes that lead to long-term illness.

That realization forced me to expand my perspective.

There is an entire spectrum of therapeutic strategies that can support human biology beyond traditional interventions. When applied responsibly and guided by evidence, they can help patients restore metabolic health, reduce inflammation, and support recovery.

These include areas such as lifestyle medicine, targeted nutrition, photobiomodulation and light therapy, regenerative medicine, peptide science, oxygen therapies, and technologies designed to support mitochondrial function.

None of these approaches replace the life-saving tools of modern medicine.

But ignoring them means ignoring powerful opportunities to improve human health.

We are living in an exponential age of technology.

I don’t get energized by easy cases.

I get energized by the ones nobody else can figure out.

The patient who has seen five doctors.
Had three surgeries.
Been told “your scans look fine.”

When someone loses faith in the healthcare system, it is rarely because of one bad test result. It is because they felt dismissed. Rushed. Minimized. Labeled.

Belief is powerful medicine.

Most medicine is still focused on structure.

This is a big deal.

On January 28, the FDA fast-tracked a new rapid Lyme diagnostic test. And if you understand how devastating chronic Lyme can be, you know why that matters.

Right now, early Lyme diagnosis is flawed. The standard ELISA and Western Blot tests rely on the body producing antibodies. The problem is that antibodies take time. There’s a three to six week window where patients can test negative — even though the infection is present.

That delay changes everything.

Lyme is known as “the great imitator.” Brain fog. Fatigue. Joint pain. Autoimmune-like symptoms. People get misdiagnosed for years. The CDC estimates 476,000 cases annually in the U.S., and that number is likely higher.

The new pocket test uses microarray technology to detect Borrelia DNA directly from interstitial fluid. No waiting on antibody production. Results in about 20 minutes.

That changes the timeline.
And in Lyme disease, timeline matters.

Fast track does not mean immediate approval. We’re likely looking at late 2026 or early 2027. But this is progress.

Earlier detection means earlier treatment.
Earlier treatment means fewer chronic cases.

That’s worth being excited about.

If you or someone you know has unexplained fatigue, neurological symptoms, or persistent pain without clear answers, push for proper evaluation. Early intervention is everything.

Nearly 50% of buildings in the United States have documented water damage. 

Mold exposure is common. Yet patients with persistent brain fog, chronic fatigue, joint pain, anxiety, skin inflammation, hormone disruption, and immune dysfunction are often told their labs are “normal.”

For individuals with certain HLA-DR genetic patterns, the body does not clear biotoxins efficiently. The result is sustained inflammation, mitochondrial dysfunction, immune activation, and progressive loss of energy production.

Most people believe knowledge is about defending what they already know.

I believe the opposite.

The more I learn, the more I realize how little we actually understand about the world around us.

There are two types of people.

The first group spends their lives expanding what they know.
They stay curious. They explore new ideas. They ask uncomfortable questions.

The second group spends their lives protecting what they already know.
They defend their current beliefs. They resist new perspectives. They treat uncertainty like a threat.

Unfortunately, in medicine we see a lot of the second group.

But progress does not come from defending the past.
Progress comes from curiosity.

One of the fields that fascinates me the most is quantum mechanics.

Quantum physics forces us to confront something uncomfortable. Reality is far more complex than our current models. Concepts like superposition, probability states, and even Schrödinger’s Cat challenge the way we think about certainty, observation, and decision making.

Last year I gave a keynote speech where I applied quantum mechanics principles to something completely different.

Sales.

Specifically how medical device representatives communicate with surgeons.

At first the audience thought it was bizarre.
Quantum physics and medical sales seem like completely different worlds.

But once you understand the underlying principles, the connection becomes clear.

Observation changes outcomes.
Possibility exists before decision.
Probability shifts based on interaction.

The same principles that govern physics often appear in human behavior, decision making, and leadership.

The real danger is not ignorance.

The real danger is intellectual rigidity.

The moment we believe we already know everything we need to know is the moment progress stops.

Curiosity is not a luxury.
It is the engine of innovation.

Most people see the surgeon.

Lab tech.

Every one of them mattered.

Those jobs taught me discipline before I ever held a scalpel.
They taught me humility before I ever had a title.

Hard work doesn’t start when you become successful.
Hard work is what builds the foundation for success.

Too many people today want the title without the struggle.
The recognition without the sacrifice.

But the truth is simple.

The operating room is not where character is built.
It’s where character shows up.

The real work happens long before anyone is watching.

Stay humble.
Work relentlessly.
And never forget where you started.

Because the journey shapes the leader.

What 10,000 Surgeries Taught Me

Learning to receive is an art.

Some people need to learn how to give.

Others of us have the opposite problem.

For years I was comfortable doing the giving.

Helping. Showing up. Supporting others. Sharing what I could.

But receiving? That felt different.

Compliments. Opportunities. Even kindness.

Sometimes when you’ve been fortunate in life, when you’ve worked hard and also been blessed along the way, accepting something back can feel uncomfortable.

 Almost like it should go to someone else who needs it more.

I’ve also learned the hard way that when you are naturally a giver, people will sometimes take advantage of that. 

That’s part of the journey too.

What I’ve come to understand is this:

Giving and receiving are part of the same cycle.

If we refuse to receive, we quietly break that cycle. We deny someone else the opportunity to give.

Grace is not only in generosity.
Grace is also in accepting with humility.

And for many of us, that is the harder lesson.

Why BPC 157 will never get FDA Approval: and why it’s not what you think. 

In late 2023, the FDA put BPC-157—a powerful, endogenous peptide that in preclinical studies accelerates tendon repair and seals "leaky gut"—on a restricted list, effectively banning it from compounding pharmacies.

The internet screamed: "Big Pharma is suppressing a miracle cure!"

As a surgeon who has spent 20 years as an insider, let me give you the forensic truth: It wasn't malice. It was math.

The FDA’s regulatory framework is built to approve single-target chemical inhibitors (like an NSAID). BPC-157 is an "Operating System Update." It heals the gut, the nerves, and the tendons simultaneously. You cannot force a systemic hardware upgrade into a regulatory pathway designed for a single software patch.

Add the fact that BPC-157's patent expires in 2033, and the math is clear: no pharmaceutical company was ever going to drop $500M on Phase III trials for a molecule they couldn't monopolize.

News broke recently that the FDA is preparing to move 14 restricted peptides—including BPC-157—back to the Category 1 list. Compounding them with a prescription will be available again, which means that more people can get expert opinions and not source them from “research” pharmacies. 

Because of patent economics, BPC-157 will likely never be a formal "FDA-Approved Drug” however it will continue to be utilized to try to help heal and recover.  Much more human research data is needed but when used under the supervision of expert providers can be a useful tool for many conditions (not medical advice).
