Last week, I sat in an Airbnb living room in London at 7am, watching 23 people show up to get their blood drawn.

Not because we offered discounts. Not because of aggressive marketing.
Because they wanted to understand their bodies.

This was OUTLIVE — Lucis' launch week in the UK.

9 events. 142 people. 78 blood draws.

But here's what the numbers don't capture:

The woman who stayed 45 minutes after her blood draw, asking about every biomarker we test. The founder who brought his entire team. The couple who registered together on Valentine's Day because they want to optimize their health as a unit.

These are not customers. They're people taking control of their biology.

LLMs score 95% on medical benchmarks. Real people using them? Under 35%.

A new Nature Medicine study tested whether ChatGPT, Llama, and Command R+ actually help people make better health decisions.

The result: they don't. Participants using LLMs performed no better than the control group at identifying conditions or choosing the right course of action.

The problem isn't the model. It's the gap.

Generic AI doesn't know your biomarkers. Your history. Your context. It gives you a textbook answer when you need YOUR answer.

The bottleneck in health AI was never model intelligence.

A Nature Medicine study from Oxford just proved it (link in the first comment).

1,298 people. A randomized controlled trial. Three top LLMs — GPT-4o, Llama 3, Command R+.

The models alone? 95% accuracy identifying health conditions.
The same models with real users? 34%. Worse than people who just used Google.

The control group had 1.76x higher odds of identifying a relevant condition than people using AI.

The researchers found the breakdown happens in three places:
 → Users don't know what's medically relevant — they give the model incomplete information
 → The model surfaces the right answer in conversation — but users don't extract it
 → Users don't consistently follow correct recommendations, even when they appear in the chat

The paper's conclusion is essentially a product spec: "For a public-facing medical LLM to exist, it would need to be proactive in managing and requesting information rather than relying on the user to have the expertise to guide the interaction."

The conclusion from Nature Medicine: we need "more reliable and deterministic conversational LLM-based tools in high-risk settings."
That sentence is essentially a product specification.
At Lucis, we've been building to it from day one — without knowing this paper would validate us.
