Health System Specialist (High Reliability Organization Lead)

• Coordinated a pilot program for Veterans with dementia & mental illness, focusing on
robust trainings, enhanced communication & individualization of care
• Managed a caseload of 10-15 complex Veterans, including providing education to Veterans &
care partners to facilitate carryover in learning
• Facilitated multiple evidence-based, therapeutic groups for Veterans with dementia, mental
illness and other disabilities (~2-4 groups/week)
• Designed a sensory room for Veterans to help with their distress behaviors
• Facilitated weekly interdisciplinary team meetings (watchlist huddle, dementia care plan/
programming, sleep committee, culture change, employee wellness)
• Managed $35,000 in program funds to ensure proper use to improve Veterans’ life
