⋆ Enhancing joint replacement pathways via in-depth analytics. 

⋆ Leading large-scale post-op anaemia project involving analysis of 1000+ patients’ data.  I have developed a new pathway (phase 3) which has been in place since Jan 2019 and significantly reduced post-op transfusion from 9% to 2.7%.

⋆ 1% upgrade of care achieved for joint replacement patients in 2019 - paving the way forward for an innovative major elective surgery model (in ASA 3 patients and below) on a site without intensive care provision.

⋆ Collaborating with digital programmes team to:
          - enhance electronic prescribing system
          - develop MDT forms so critical information can be shared with a neighbouring hospital's specialist team
          - supporting transition to paper-free electronic patient record (EPR) by helping to implement active clinical notes (ACN)

⋆ Continuing to support efficiencies established during the NHS 100 day challenge initiative in 2019.  The primary focus is on maintaining a reduction in elective care cancellations (95% reduction in Jan 2019)

⋆ Introduced performance dashboard displaying key metrics for staff on the ‘shop floor’ 

⋆ Established outpatient follow-up pathway for patients with infected joint replacements who are discharged home on intravenous antibiotics to ensure they are closely monitored

Elective Care Specialist Registrar

Responsible for peri-operative management of patients undergoing primary/revision knee and hip replacement surgery.  

Led and implemented multiple change and efficiency initiatives:

⋆ 50% reduction in LOS (length of stay) of joint replacement patients achieved through efficiencies in workflows/patient pathways.
⋆ Continuous biannual evaluation of 30 day readmission.  Identified that at least 50% of readmissions were preventable and to the wrong clinical areas.  An ad hoc ward based clinic was established alongside an automated weekly readmission report.
⋆ 90% reduction in prescribing time achieved for joint replacement - collaborated with digital programmes team and informatics pharmacists to develop bespoke drug order sets facilitating safe consistent electronic prescriptions.

⋆ Elimination of medication-related delays in discharge achieved by introducing a system for ordering discharge medications pre-admission.
⋆ Improved transfusion documentation by developing a ‘decision to transfuse sticker’ as a joint venture with blood transfusion co-ordinator.  Consequently, a similar sticker has been introduced nationally.
⋆ Reduced post-op delirium by collaborating with psychiatry liaison team, and lead lower limb anaesthetist to develop tailored post-op pain management regime for high risk patients.

⋆ Authored specialist joint replacement handbook for healthcare professionals to ensure standardisation of care
⋆ Authored SOP for non-medical authorisers of blood products in elective care
⋆ Authored venous thromboembolism guideline for lower limb arthroplasty
⋆ Established weekly teaching to upskill staff
