Managed the clinical liaison department regarding evaluating potential patients and transmission of information to CMS and UDS. Managed the implementation and planning of comprehensive community relations and community education programs. Responsible for the delivery of professional rehabilitation services care and provided leadership to the nursing, therapy, and liaison staff. Developed and enforced guidelines of medical plan of care and departmental policies to achieve outstanding patient care. Delivered rehabilitation services leadership and personnel management. Functioned as a physician resource to assist with documentation compliance by creating valuable documentation templates.

MANAGER OF POST ACUTE REHAB SERVICES
Responsible for the activities, supervision, management, and direction of the rehabilitation services department in accordance with organizational and departmental philosophy and goals. Assist the director in maintaining the highest degree of clinical and operational performance of the department. Responsible for the leadership of rehabilitation services personnel and/or students and functions as a resource for physicians and patients. Responsible for the referral and business development and the performance outcomes management of post acute rehabilitation services including the inpatient acute rehabilitation program and the outpatient rehabilitation program. Work closely with the rehab director and the medical director of the inpatient acute rehabilitation program, oversees the pre-admission patient assessment screening assessment process and provision of clinical assessments of potential clients for the review and decision making about admission to the medical director. Oversee the IRF-PAI and transmission of information to CMS and UDS. Oversee the clinical assessment coordinator with all of the duties. Oversee the implementation and planning of comprehensive community relations and community education programs. Oversee the inpatient and outpatient rehab departments internal and external community relations and communications. Responsible for the delivery of professional rehabilitation services care and provide leadership to the staff. Develop and enforce guidelines of medical plan of care and departmental policies.

Accountable for daily screening of 8 to 10 patients. Managed multiple tasks to ensure the rehab program was in compliance with federal and state regulations. Conducted the pre-admission patient assessment screening assessment and provided clinical assessments of potential clients. Responsible for the completion of the Inpatient Rehab Facility Patient Assessment Instrument (IRF-PAI). Reviewed medical records to ensure appropriate team documentation occurred to support findings noted on the IRF-PAI and to meet medical necessity as defined by CMS regulations.

Leadership and management responsibility for planning and implementing the programs and services of the department and ensuring that they are consistent with the organization’s mission and strategic plan; in accordance with organizational policy and standards; compliant with regulatory, billing, and accreditation requirements; and within established department goals and resources. Work with the vice president to establish operational, financial, quality, and service goals for the department and ensures that performance is monitored and appropriate and prompt corrective action is taken in support of achieving those goals. Work closely with nursing leadership to ensure that the nursing care provided in the department integrates effectively into the overall inpatient rehabilitation program and meets patient needs. Establishes and maintains effective working relationships with the medical staff (including the department’s contracted physicians), organizational leaders, other departments, and the department’s own staff. Work with the department’s leadership team and staff to foster a collaborative environment that enables the department to achieve its goals.
