•	Will implement CHBC activities as per GF guidelines. 
•	Will manage project at CHBC site with close coordination with program manager and PACP.
•	Will develop and maintain referral mechanisms with relevant partners.
•	Will maintain the attendance register and ensure timely payment of salary for CHBC Staff.
•	Will ensure high quality care and support services are offered regularly to the patients admitted in CHBC program.
•	Will ensure diet requirements are provided to the patients as per the clients Chart and sample and the food being supplied.
•	Will ensure good patient up-take by establishing linkages in the ART Centre, PPTCT.
•	Will ensure high quality counseling services provision to PLHIVs.
•	Will examine the patient’s record and the maintenance of the same and the administration of the drugs.
•	Will examine the client’s record for any referral made for medical support services and the outcome of such referral.
•	Will ensure that the minimum space as well as equipment and communication material required for CHBC is in place at all times.
•	Will supervise the functioning of the CHBC through monthly staffing review with the staff.
•	Will ensure data quality checks to ensure accuracy of data generated by CHBC Staff by Checking with the registers and records maintained by the CHBC
•	Will ensure that monthly/quarterly reports are sent in timely manner to funding agency.
•	Will ensure that all Staff at the facility are sensitized on the package of the services available for the control and prevention of HIV so as to build ownership to remove myths and misconceptions and prevent instances of stigma and discrimination.
              Any other task assigned by supervisor.

•	Provide NRC beneficiaries with accurate and updated information on the legal and administrative assistance available through the ICLA program;
•	Participate on regular overnight field visits to NRC beneficiaries throughout area of responsibility; 
•	Register and process administrative requests submitted by NRC beneficiaries;
•	Review and prepare administrative requests for referral to other organizations
•	Monitor and report on the progress of administrative request referred to other organizations/IPs offices in the province;
•	Maintain updated contact information on all NRC beneficiaries who have submitted legal claims and/or requested administrative assistance by the NRC;
•	Maintain updated contact information on all community representatives of NRC beneficiaries throughout the area of responsibility;
•	Maintain a high level of personal competence on all relevant fields governing the ICLA mandate, and actively participate in internal information coordination meetings to enhance and facilitate the implementation of the ICLA activities in Pakistan; 
•	Write regular activity reports in the designated areas of responsibility.

With these rich and dynamic experiences and theoretical knowledge, I am committed to building and transforming marginalized communities to become the driver of their own destiny.
