Working as part of the North Sedgemoor Primary Care Network providing non clinical patient care through 1-2-1 supportive sessions and community groups. I offered a variety of patient care through a TGROW coaching model from patients looking to make health improvements. Furthermore, I support patients through crisis interventions, domestic's violence DASH assessments and early help assessments. Likewise, I worked closely with the other members of the primary care team and complex care teams in the management / decision making about care and service provision for individual patients.

Wessex Learning Trust

As a Parent and Family Support Advisor: Service Development Lead, I was recruited by the Wessex Learning Trust to design and implement a new service to provide support to 8 school across a multi academy trust. The role enabled me to develop the service into something accessible to those most at needs. Ensuring all support was documented and family's and co-workers safeguarded through enhanced communication and accurate recording system. Working closely with other professional to ensure a seamless service was implement and early help access for families and pupils across the Wessex Learning Trust. Working with families affected at level 2, through to level 4 from behavioural SEN needs to high level safeguarding concerns.

Rethink Mental Illness

As MHRW I supported mental health recovery within a 24 hour intensive housing service, carrying out individual tailored support to enable clients to practically address their specific needs, goals and assist them to move onto independent living. I managed a large case load of clients affected by acute mental illness, ensure all outcomes of support were met and carried out by assistant mental health recovery workers within the team. Likewise, I would work alongside the clients to write tailored support plans, risk assessments and regularly taking part in client's reviews with the NHS Somerset Partnership. The service supported a structured and an empowering environment to adults experiencing high needs mental health issues: The Mental Health Recovery Star. Working with clients on a 1-2-1 basis providing a range of housing-related support in addition to supporting them to develop their basic living skills, develop confidence, self- esteem and regain control of their lives. Furthermore, I regularly liaise with partner agencies, housing providers and
 
voluntary organisations so I am able to signpost clients to the best services and facilities for their specific and individual needs.

Recovery Practitioner (MHRW)

Knowle West Media Centre

The role was to coordinate and deliver a £250,000 Children In Need funded project within KWMC, to young children, in need, throughout South of Bristol. My role was to recruit and engage young people into the projects on offer at KWMC, and build and develop professional relationships with young people participating in the project.
Additionally, I supported KWMC Education Producer to deliver media workshops in primary and secondary schools, in Bristol and North Somerset. Through the work I aimed to develop young people’s self confidence, self esteem and aspirations using creative youth work and media tools. Delivering workshops in accordance to project plans and funding aims and objectives. Making sure to plan, deliver, monitor and evaluate youth work sessions and the wider KWMC projects. I ran weekly media sessions for a range of projects also working with media professionals to deliver fun engaging sessions. Through these sessions I would assess the most appropriate form of accreditation for each young person in accordance to engagement level and age.
