the big issue with health insurance in the US is churn - every payer assumes you're going to leave in ~2 years so they don't really care about paying for things upfront or preventive care

what if we had a system similar to carbon credits but for payers, where if they paid for preventive things they get credits and if they don't, they need to buy credits from another payer

I know, administratively complex. But could potentially solve the issue around not trying to pay for preventive issues that might help everyone downstream

I've been bingeing many of the Acquired episodes and one thing that's stuck out to me is how weird the structures are for some of these companies AND how critical that specifically was to their success. 

Examples:

Visa: Got banks to participate in the network with a non-transferable profit sharing scheme that was relative to the amount of transactions they brought to the network

Novo Nordisk: A foundation owns the operating company, and to this day owns the majority of voting shares

Walmart: Each store was it's own company and people could own shares in each store individual. Every manager owned shares in their own store + get profit sharing incentives for their store + invest in the other stores to align incentives with other managers

Sometimes I think new companies don't try to experiment enough with organizational structure because the boilerplate docs don't allow for it, or they don't want to spend time reinventing something that's not their core competency.

But I actually think these choices could be the differentiating factor AND actually makes companies more durable in the long run
