I am involved in the management for older patients encompassing the diagnosing, treatment and prevention of multiple complex chronic medical and mental conditions.

A major part of the role involves screening, assessing and stratifying of patients with geriatric syndromes while developing treatment plans to manage diverse complex geriatric syndromes including frailty, sarcopenia, functional decline, falls, incontinence, malnutrition, cognitive impairment, mobility & balance, osteoporosis, osteoarthritis and polypharmacy. 

This includes coordinating a team of allied health specialists covering diverse medical and surgical disciplines in deploying holistic approach in managing health conditions to improve the quality of life of older patients.

Besides reviewing and increasing geriatric services offered covering falls clinic and memory clinics, I also identify and implement improvement opportunities to reduce patient waiting time and enhance efficiency.

Lastly, I facilitate periodic continuous medical education and knowledge sharing sessions.

Geriatrician

Geriatric Fellow

As a Specialist, I performed assessment and admission of patients with acute medical conditions across the intensive care unit, cardiology care unit (CCU) and cardiology ward.

Besides that, I also managed in-patient cases including investigations ordering and coordination, medical history and condition examination, daily condition monitoring and documentation along with discharge planning.

Lastly, I liaised with all members of the healthcare team to ensure effective patient management.

Internal Medicine Physician

Here, I performed various medical functions across the Dermatology, Geriatric, High Dependency Ward, Nephrology as well as Hematology units.

This included conducting diagnosis of illnesses and development of treatment plans and prescription of medicines while referral critical cases to specialist departments within the hospital.

Additionally, I carried out minor surgical procedure including incision and drainage of abscesses, wound suturing and removal of foreign bodies in the Emergency Department.

Lastly, I provided counselling and medical education to enhance the wellbeing of patients.

Medical Officer Department of Medicine

During this tenure, I carried out diagnosis and investigation of patient symptoms including assessment of medical and family history evaluation as well as evaluation of laboratory test results and radiology imaging.

Besides that, I provided assistance during surgeries and deliveries while performing ward rounds and bedside procedures.

Additionally, I worked closely with nurses, social workers, rehabilitations therapist, pharmacists and other health care providers towards ensuring excellent patient care.
