I am involved in the management for older patients encompassing the diagnosing, treatment and prevention of multiple complex chronic medical and mental conditions.

A major part of the role involves screening, assessing and stratifying of patients with geriatric syndromes while developing treatment plans to manage diverse complex geriatric syndromes including frailty, sarcopenia, functional decline, falls, incontinence, malnutrition, cognitive impairment, mobility & balance, osteoporosis, osteoarthritis and polypharmacy. 

This includes coordinating a team of allied health specialists covering diverse medical and surgical disciplines in deploying holistic approach in managing health conditions to improve the quality of life of older patients.

Besides reviewing and increasing geriatric services offered covering falls clinic and memory clinics, I also identify and implement improvement opportunities to reduce patient waiting time and enhance efficiency.

Lastly, I facilitate periodic continuous medical education and knowledge sharing sessions.

As a Specialist, I performed assessment and admission of patients with acute medical conditions across the intensive care unit, cardiology care unit (CCU) and cardiology ward.

Besides that, I also managed in-patient cases including investigations ordering and coordination, medical history and condition examination, daily condition monitoring and documentation along with discharge planning.

Lastly, I liaised with all members of the healthcare team to ensure effective patient management.

Here, I performed various medical functions across the Dermatology, Geriatric, High Dependency Ward, Nephrology as well as Hematology units.

This included conducting diagnosis of illnesses and development of treatment plans and prescription of medicines while referral critical cases to specialist departments within the hospital.

Additionally, I carried out minor surgical procedure including incision and drainage of abscesses, wound suturing and removal of foreign bodies in the Emergency Department.

Lastly, I provided counselling and medical education to enhance the wellbeing of patients.

During this tenure, I carried out diagnosis and investigation of patient symptoms including assessment of medical and family history evaluation as well as evaluation of laboratory test results and radiology imaging.

Besides that, I provided assistance during surgeries and deliveries while performing ward rounds and bedside procedures.

Additionally, I worked closely with nurses, social workers, rehabilitations therapist, pharmacists and other health care providers towards ensuring excellent patient care.

It was an honour 😊 to take part in a caregiver support sharing session for individuals with dementia under the Alzheimer Disease Foundation Malaysia last weekend. 
 
Caregiving, though crucial, can put considerable stress 🌡️ on the caregiver, leading to what is known as caregiver burden. 
 
This term lacks a universally accepted definition. However, several researchers link it to elements such as psychological distress 😔, physical health problems 🤕, financial difficulties 💰, or the impact on interpersonal relationships 💔 arising from caregiving duties. 
 
These may include feelings of overwhelmingness, regular tiredness 😴, weight fluctuations ⚖️, feelings of loneliness or worry 😞, irritability, sleep disruption, loss of interest in activities once enjoyed, physical discomfort, and increased dependence on substances such as alcohol, drugs, or tobacco 🚬. 
 
Continual caregiver stress can harm health, especially when experienced over a prolonged period. Caregivers may neglect their dietary habits, physical activity 🏋️, and sleep 😴 due to their duties, leading to potential health problems. These can range from diabetes, cancer, arthritis, heart disease, to mental health concerns like anxiety and depression. 
 
Recent studies reveal that a substantial number of caregivers disregard their health: within a month, approximately 14.5% reported experiencing mentally unhealthy days and 17.6% reported physically unhealthy days. In addition, nearly 36.7% reported less than seven hours of sleep in a 24-hour period, with 40.7% reporting suffering from two or more chronic diseases. These figures highlight the urgency 🚨 to address these health issues given the growing caregiver population and mitigate the risk of caregiver burden.

Mrs. M, an indomitable woman of 80 years.  
 
A life-long jazz lover, M carried an aura of exuberance that dementia hadn't managed to extinguish. However, her refusal to eat had become a daunting hurdle that was increasingly distressing for her family. 
 
Once a woman with an insatiable love for food, M now sat stony-faced at the dining table, refusing to eat, eyes vacant.  
 
Her sudden disdain for food had become a grave concern, causing her health to falter. No amount of coaxing or tempting with favourite dishes seemed to work.  
 
The family was spiralling into distress, feeling helpless in the face of M's deteriorating health. 
 
J, M's eldest son, decided to seek external help. He organised a family gathering, inviting everyone M held dear, hoping that familiar faces around the table would stimulate her appetite.  
 
Despite the house buzzing with laughter and affection, M sat silently, oblivious to her surroundings, untouched plate of food lying cold in front of her. 
 
As desperation began to grip J, an idea sparked. From the corner of his eye, he saw an old record player gathering dust in the corner. It was a relic, a vintage piece from M's past.

J, seizing this fleeting moment of clarity, took his mother's hand in his, leading her into a slow dance around the room.
